Return Opened Intraocular LensesTo return any OPENED lenses which were not used, had contact with the patient, or the subject of a possible complaint, please complete a Return Authorization (RA) form. There is no need to fax the RA form in advance.
Please complete the second page of the form if the lens has had contact with the patient and contact Customer Service for a RA# and prepaid label.
Return overstock, Consignment, OR Expired lenses, & InjectorsIf you are returning an UNOPENED overstock, expired/close to expired, or consignment lenses or injectors, please complete an Inventory Return (IR) form and return the completed form and lenses.to the return address below. There is no need to fax the form in advance.
Note: Credits from returned lenses may only be applied to future purchases as we do not issue refunds for credits on the account.
